**Indicates your insurance requires a referral from your primary care physician (PCP) to be seen by a specialist.

Referrals/Pre-Authorizations – Some insurance plans (see above **) require you to obtain referral or authorization from your primary care specialist (PCP) before seeing a specialist’s office, such as ours. Without a referral on file, you could be subject to higher out-of-pocket charges or costs may not be covered at all. While we are happy to assist in coordinating with you and your PCP to receive this referral, it is ultimately the patient’s responsibility. It is important that you fully understand your insurance plan’s rules for seeing a specialist. For more information, call the member services phone number on the back of your insurance card. Our office is happy to assist you if you have further questions regarding your referral or pre-authorization.
